  Reason: To omit “C14023 C14024” and insert “C14229 C14260” in numerical order in the column headed “Circumstances” for the entry Mesalazine in the form Tablet 1 g (prolonged release) [Maximum Quantity: 120; Number of Repeats: 5] in Part 1 of Schedule 1 as the amendments were not incorporated during the compilation process. <br><br> To reinsert the entry for Methotrexate in the form Injection 5 mg in 2 mL vial in Part 1 of Schedule 1 as the entry was incorrectly substituted during the compilation process. <br><br> To substitute the entry for Methotrexate in the form Injection 50 mg in 2 mL vial in Part 1 of Schedule 1 as the amendment was incorrectly incorporated during the compilation process.
